No-Exam Life Insurance: How It Works & Who Qualifies

No-exam (also called “accelerated underwriting” or “simplified issue”) life insurance lets qualified applicants skip the paramedical exam — no blood draw, no urine sample, no in-home appointment. Approval can come in as little as 15 minutes to 72 hours instead of 4–8 weeks. Three flavors of “no exam”

  • Accelerated underwriting (AU) — fully underwritten pricing, no exam. The carrier uses data sources (MIB, prescription history, MVR, electronic health records) to decide your rate class. Same premium as traditionally underwritten. Best healthy-applicant choice.
  • Simplified issue (SI) — a short health questionnaire, no exam, but premiums run 20–60% higher than fully underwritten. Useful when you don’t qualify for AU.
  • Guaranteed issue (GI) — no health questions, anyone in the age band is approved. Very expensive, low face amounts ($5K–$25K), and a 2- or 3-year graded death benefit. Final-expense use only.

Who qualifies for accelerated underwriting (no exam, no penalty)

Typical AU eligibility windows across major carriers (Pacific Life, Lincoln, Protective, John Hancock, Symetra, Mutual of Omaha):

  • Age 18–60 (some go to 65)
  • Face amount up to $1M–$3M depending on carrier
  • Build within the carrier’s height/weight chart for Preferred or better
  • No tobacco/nicotine use in past 12 months
  • No major health conditions (cancer, heart disease, diabetes with complications, recent stroke, ongoing treatment for mental health hospitalization)
  • Clean MVR — no DUI in past 5 years, no more than 2 minor violations
  • No bankruptcy in past 24 months
  • U.S. citizen or permanent resident

What the carrier checks (with your consent)

  • MIB (Medical Information Bureau) report
  • Prescription history (Rx databases such as ScriptCheck, MIB Plus)
  • Motor Vehicle Report (MVR)
  • Electronic Health Records (EHR) for participating providers
  • Public records — bankruptcies, criminal history
  • Sometimes a phone interview (15–30 minutes) to confirm answers

The trade-offs nobody mentions

  1. If you have any flag (high cholesterol, elevated A1C, mental health prescriptions, two speeding tickets), the AU algorithm may kick you to full underwriting anyway. You won’t know until after you apply.
  2. Maximum face amounts are lower — if you need $3M+, you’ll likely need a traditional paramedical exam regardless.
  3. Simplified issue prices are not competitive. If you’re healthy, applying for SI when you’d qualify for AU costs you money for the life of the policy.
  4. Guaranteed issue is last-resort coverage only. Most GI policies pay only return of premium plus interest in years 1–2 if death is from natural causes.

Best-fit scenarios

  • Healthy applicant, $250K–$1M, age 30–50 — accelerated underwriting is usually a no-brainer.

  • Diabetic on insulin, recent cancer, smoker — fully underwritten or simplified issue likely outperforms AU.
  • Age 65+, final-expense need — guaranteed issue or simplified-issue final expense from carriers like Mutual of Omaha, AIG, or Royal Neighbors.

Texas-specific notes

Texas requires carriers to deliver a written notice of any adverse underwriting decision and the consumer’s right to request the source data under Texas Insurance Code Ch. 559. If you’re declined or rated, request your MIB file and Rx report (both free annually) before applying anywhere else — an incorrect entry can follow you for 7 years.
